Village Medical Stay Well at Home

Our priority is to help you get well and stay well. That’s why we are offering remote patient monitoring through the Village Medical Stay Well at Home program for our patients with conditions such as congestive heart failure. The program provides ongoing support for your condition between office visits and from the comfort of your home.

What is the Village Medical Stay Well at Home program?

The program is designed to help you and your Village Medical doctor or advanced practice provider* (APP) monitor your chronic condition(s) on a daily basis. The monitoring is done by using simple devices to measure your health readings at home and automatically send them to us. Your care team will then check these readings to:

- Catch health changes early.
- Reduce trips to the hospital.
- Better manage your ongoing health conditions.
- Help you feel better day-to-day.

1. Getting Started

Your Village Medical doctor or APP will recommend the program for you if they think it could have a positive impact on your health. You will then be enrolled in the program. We are working with Cadence to provide you with an easy-to-use monitoring device at no extra cost.

2. Daily Checks

You will take your health readings from the comfort of your home using the device. For patients with heart conditions, that would include a blood pressure monitor and/or weight scale.

3. Automatic Updates

Your readings are automatically and securely sent to your doctor or APP and the care team.

4. Personal Support

The care team will check in with you regularly by telephone for ongoing support, education and to answer any questions you may have. If any of your readings show a potential concern, they will contact you right away for the next steps.

Does insurance cover this? 


Remote patient monitoring is a covered benefit under Medicare and other commercial insurance plans. After deductibles are met, the majority of patients experience little to no cost. We encourage you to verify your benefits coverage directly with your insurance provider.

What device will I receive?

Depending on your health needs, you may receive: 

  • A blood pressure cuff (for high blood pressure or heart conditions)
  • A weight scale (for heart health)

How do I sign up? 


If your doctor or advanced practice provider recommends this program, we will call or text you to enroll.

Is my health information safe? 


Yes. All your health information is protected and secure.
